Fee Schedule/Direct Care/Insurance

Financial Policy

At the Institute of Dermatology and Oculoplastic Surgery, we strive to provide excellent medical care to our patients while maintaining financial transparency. As a brand-new practice, we are currently contracted with Medicare and Cigna, but there are other insurance companies with which we have yet to secure an agreement. However, we want to assure you that we are committed to helping you with your medical needs regardless of your insurance carrier.

If we are not contracted with your insurance company, you will be responsible for the cost at the time of service. As a courtesy, we will submit the claim to your insurance company, and if the claim is accepted, you will be issued a credit or reimbursement. Please note that not all insurance companies allow reimbursement for out-of-network providers, so it is important to check with your insurance provider regarding their policies and rules regulating reimbursement.

For patients who do not have medical insurance or prefer not to use their insurance, we offer a direct care model. With this model, there are no surprise bills, hidden fees, co-pays, or insurance cards. You simply stop in, get a service, and pay for it, meaning straightforward medical care when you need it.

We believe in cost transparency, and a list of pricing is available on our website to provide you with a good-faith estimate of your charges for the visit. Flexible payment options are available, including using your Flexible Spending Account (FSA), a flexible spending arrangement, or Health Care Savings Account to pay for your care. For those with private insurance, we submit the claim to your insurance as a courtesy. Alternatively, we can provide patients with an itemized and coded form and the appropriate out-of-network healthcare reimbursement form to submit to the patient’s insurance company for reimbursement (upon request).

We understand that navigating medical insurance and payments can be overwhelming, but we are here to help. Please feel free to call our office to schedule an appointment or book one online today. Thank you for choosing the Institute of Dermatology and Oculoplastic Surgery for your medical needs.

Important

We value our relationship with you and want to keep you as our patient. To this regard, we will keep the rates of our direct pay patients as reasonable as possible while still delivering the quality of care you deserve.

PAYMENT AT TIME OF SERVICE:

Payment is due in full at the time of service unless you are covered by Medicare or an insurance company with which we are contracted. You will be charged a $50 service fee for any returned checks, with no exceptions.

Credit Cards on File will be obtained from all patients. This is due to an era of medicine where we are working with high deductibles that are not readily apparent to the clinics. The credit card on file is only used in the event of a delinquent balance or for refunds. If invoices are not paid within 2 weeks, the credit card on file will be charged the remaining balance.

At the Institute of Dermatology and Oculoplastic Surgery, we believe in providing transparent financial policies to ensure that you receive the care you need without any surprises.

Fee Schedule:

Our fee schedule is a general guideline, and your charges may vary depending on the complexity of the specialized service you receive and the anatomic location. We strive to keep our rates reasonable for our direct pay patients while maintaining the highest quality of care.

Insurance Coverage:

We are contracted with Medicare and Cigna. If we are not contracted with your insurance provider, you may still be able to file for out-of-network benefits. We can file your insurance claim as a courtesy.

Biopsies and Excisions:

In some cases, biopsies and excisions may require confirmatory microscopic analysis, which may generate additional charges from skin pathology experts. For patients with insurance, we use an in-network reference laboratory. The self-pay rate for pathology is $69.52 for basic stains.

Payment Policy:

Payment is due in full at the time of service, except for patients covered by Medicare or contracted insurance companies. We accept various payment methods, including credit cards, cash, and checks. We do require a credit card on file to ensure that delinquent balances are paid on time, and we charge a $50 service fee for any returned checks.

Credit Cards on File:

We obtain credit card information from all patients to ensure that we can collect any outstanding balances efficiently. Your credit card on file will only be used in the event of a delinquent balance or for refunds. If you have an unpaid balance after 3 weeks, we will charge the remaining balance to your credit card on file.

We hope that our transparent financial policies give you peace of mind when seeking care at our dermatology and oculoplastics practice. Please don’t hesitate to contact us if you have any questions or concerns.
